This form must be completed in its entirety and signed before it is entered on a student’s record.  Some browsers may require saving rather than opening this document.  This form can be completed online prior to printing.

Please note:  Students with F-1/J-1 visas are reminded that that they must also contact UConn’s Immigration Services to extend their Form I-20/DS-2019 to correspond with extended deadlines.
