Multiple Clinic Request Form

This form is to be used by students enrolling in multiple clinics in one semester to acquire the consent of clinic instructors. As conflicts pertaining both to clients and time commitments, may occur, students must receive prior permission to participate concurrently in two clinics in the same semester. Grading of clinics is done a graded basis for the classroom or seminar component of the course; fieldwork and clinical externships are done on a pass/fail basis only. Students should consider that there is a limit on the number of pass/fail credits that may be obtained during their course of study. Please also note: only upper division students may enroll in clinics.
