Instructions and Deadlines

To comply with the regulations established by the Immigration and Naturalization Service, the University of Connecticut School of Law is required to submit the following for all persons who are not citizens of, or do not have permanent alien status in the United States of America. This form must be returned before any admission decision can be offered on a student application, or before requests from visiting or exchange international scholars can be processed.

This form is to be used ONLY by students and scholars applying to the University of Connecticut School of Law. Applications materials for other University of Connecticut programs will require a separate submission of this material.

Please be advised: additional information may be required. Additional data request forms will be provided to you when available.

Explanation of Terms Used

Name
Please indicate your full name as it appears on your passport or other official identification documents. The use of a Middle Name is not required.
Gender
Please indicate Male or Female.
Date of Birth
Please indicate your birth date in the following format: MM/DD/YYYY. Example, students born on October 28, 1965 should enter 10/28/1965.
United States Social Security Number
This is not a required field, please enter this number only if known.
LSAC ID Number
This field is for JD applicants only that have been provided an identification number by the Law School Admission Council (LSAC). This is not a required field.
Address and Phone Number in Country of Permanent Residence
Unless you can produce documentation indicating you are a permanent resident of the United States, please indicate an address and phone number in the country where you currently reside.
Mailing Address in the United States
INS regulations stipulate that you may not provide a Post Office Box address as your mailing address in the United States. Please indicate a street address.
Current Visa Status
Please indicate your visa status (F-1, J-1, M-1, H-1B, or Other). If Other, please specify. Visiting, Exchange Scholars are generally on J-1 Visas. Students are generally on F-1 Visas. An H-1B is an employer-sponsored Visa. Your Visa Status may differ, these examples are only intended as guidelines.
Purpose of Travel in the United States
Please indicate Student, Visitor, Permanent Resident of the United States, or Other. If Other, please specify.
Degree Sought
Please indicate one of the following only:
  1. Juris Doctor
  2. Master of Laws in US Legal Studies
  3. Master of Laws in Insurance
  4. Exchange Student
  5. Non-Degree Student
  6. Non-Student.
Intended Start Date
Please indicate the month and year that you intend to commence studies (or your visit) at the University of Connecticut School of Law. For example, if you were beginning your studies for the Fall 2003 Semester, you would indicate: August, 2003.
Intended Completion Date
Please indicate the month and year that you intend to complete your studies (or your visit) at the University of Connecticut School of Law. For example, if you were ending your studies in the Spring 2004 Semester, you would indicate July 2004.
Financial Information
These questions are for student applicants only. Before an I-20 can be produced, a student is required to submit documentation indicating the ability to meet all financial requirements of the program. Please indicate all sources of funding, specifying the source.
