Residency Affidavit

All applicants must complete the Residency Affidavit. Once completed please save the document in one of the following supported formats: DOC, HTM, HTML, TXT, WP, WPD, DOCX. Please note that PDFs are not supported by the electronic application service because they may contain information that cannot be merged with other applicant documents. Once the Residency Affidavit is completed and saved please upload the document into the Attachments section (Residency Affidavit Form) of your electronic application.
